Changing Date, Time, and Phone Number Format to U.S.?

Can someone point me to the documentation that would allow me to change the formats for the date, time, and phone number?

Sorry, I've just found out how to do this. In admin, go to "edit form fields" then "edit field options".

uploding of small jpegs has continued to fail.  As well as support team attempts to fix the situation.  Hopefully they will be able to tinker around and fix that up.  I will post a final outcome here for anyone who may be able to use the information.
